Mental Health Therapy Services (Jamie Van Auken)

Jamie provides mental health therapy services exclusively to residents of Oregon and Washington. Services may be provided:

  • In-person: At our Portland, Oregon clinic

  • Telehealth: For Oregon and Washington residents only

By engaging Jamie's services, you acknowledge that:

  • Jamie is licensed only in Oregon and Washington

  • Services are only available to residents of these states

  • You must be physically located in Oregon or Washington during telehealth sessions

  • Services are provided under clinical supervision as required by state law

  • Your care is subject to Oregon and Washington mental health practice laws

Establishing a Patient-Provider Relationship

A patient-provider relationship is established when:

  • You complete our intake forms and provide medical history

  • Dr. Matt conducts an initial consultation or evaluation

Jamie provides telehealth services only to Oregon and Washington residents. By engaging telehealth therapy services, you:

  • Confirm you are physically located in Oregon or Washington at the time of each session

  • Agree to be in a private, confidential location

  • Understand that emergency services may be more difficult to access during telehealth sessions

  • Have backup contact information in case of technical difficulties or emergencies

  • Understand limitations of telehealth compared to in-person sessions

Telehealth Limitations

You understand that telehealth:

  • May not be appropriate for all situations or conditions

  • Requires adequate technology and internet connectivity

  • May be limited by technical difficulties beyond our control

  • May not allow for the same level of physical examination as in-person care

Appointment Scheduling, Cancellations, and No-Shows

Scheduling

All appointments must be scheduled through The Buddha's Medicine's Practice Better EHR system. By scheduling an appointment, you agree to:

  • Be prepared or arrive on time for all types of appointments

  • Have completed all required intake paperwork before your first appointment and subsequent appointments as appropriate

  • Provide accurate contact and insurance information

We reserve the right to:

  • Cancel or reschedule appointments with reasonable notice

  • Decline to provide services to any individual

  • Terminate the therapeutic relationship with appropriate notice and referrals

Payment, Billing, and Insurance

Fees and Payment

You are responsible for payment of all services rendered. This includes:

  • Session fees (medical consultations, therapy sessions)

  • Procedures or additional services

  • Products purchased (herbal supplements, etc.)

  • No-Show/Late/Cancellation fees when applicable

Insurance Superbills

Out-of-Network Services: If we are out-of-network with your insurance plan, we can provide a superbill (detailed receipt) that you may submit to your insurance company for potential reimbursement. We cannot guarantee your insurance will reimburse for out-of-network services.

Professional Boundaries

Therapeutic Relationship

For mental health therapy services, we maintain professional boundaries to ensure ethical, effective care:

  • Brief email or phone contact for scheduling or administrative matters

  • Emergency contact as previously arranged with your therapist

Inappropriate Contact:

  • Social media friend requests or following your therapist's personal accounts

  • Personal gifts of significant value

  • Social or business relationships outside of therapy

  • Contact for purposes other than therapeutic care

Dual Relationships: We avoid dual relationships (therapist-client relationship + another relationship such as friendship, business partnership, etc.) as they can compromise the therapeutic relationship and are typically prohibited by professional ethics codes.

Medical Relationship

For medical services, professional boundaries include:

  • Scheduled medical appointments

  • Appropriate clinical communication regarding your care

  • Professional, respectful interactions

We do not:

  • Prescribe controlled substances for pain management as our practice focus is integrative and lifestyle medicine

  • Provide medical care to family members or close friends due to ethical concerns

  • Engage in dual relationships that could compromise medical judgment

Discharge and Termination

Our Right to Terminate

We reserve the right to discharge you from our practice if:

  • You repeatedly miss appointments or fail to provide adequate cancellation notice
