Inappropriate Contact:
Social media friend requests or following your therapist's personal accounts
Personal gifts of significant value

Scheduled medical appointments
Appropriate clinical communication regarding your care
Professional, respectful interactions
We do not:
Prescribe controlled substances for pain management as our practice focus is integrative and lifestyle medicine

Provide written notice when possible
Offer appropriate referrals to other providers
Provide a reasonable period for you to secure alternative care
Transfer records as appropriate with your authorization
Your Right to Terminate
You may discontinue services at any time. We recommend:
Discussing your decision with your provider when possible
Attending a final session for appropriate closure (particularly important in therapy)
Requesting copies of your records if desired
Obtaining referrals to other providers if continuing care is needed

With your written authorization, we may coordinate your care with other healthcare providers. This may include:
Communicating with your primary care physician
Consulting with specialists involved in your care
